Experience: HT for 1 year or TIC for 2 years or MT for 2 years or Teacher for 5 years
Training: 40 hours of relevant training
Eligibility: RA 1080 (Teacher)

- Performance Rating for the last 3 rating periods (Attach IPCRF duly signed by proper authorities)
- Teaching Positions: SY '20-21 SY '19-20 SY '18-19
- Non- Teaching Positions (for applicants within the Division of NegOcc) CY '21, CY '20, CY '19
- Non- Teaching Positions (for applicants Outside the Division of NegOcc) last 3 years performance rating relative to the last 3 years of experience
- Certificate of training after the recent promotion/appointment
- Other designations (with specific duties and responsibilities) if any
- Position Description form for Regular Employees and from Other Government Agencies
- Omnibus Certification of authenticity and veracity of all documents submitted, signed by the applicant
- Late documents shall not be evaluated
- No additional documents shall be submitted after the deadline
- No retrieval of folders shall be allowed once received by the Districts/ Division Office
