 Be part of DSWD Field Office VI! We are currently looking for interested and qualified applicants and submit their requirements as follows: 

1. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) and Work Experience Sheet (CS Form 212) 

2. Performance rating in the last rating period or its equivalent;

3. Photocopy of certificate of eligibility/rating/license;

4. Photocopy of Transcript of Records and Diploma; and

5. Other documents supporting your credentials (e.g. training certificates, awards certificate received and etc.)
