For DSWD Region IV-A Applicants:

However, we further advise that the following documents be on hand and ready once requested:

1. Original and 2 Certified True Copy of the following:

      a. Transcript of Records

      b. Diploma 

      c. Authenticated copy of Civil Service Eligibility report of rating and/or PRC Board/Bar Rating Certificate (If applicable)

2.   Fully accomplished Personal Data Sheet (PDS) with recent passport-size picture (CS Form No. 212 Revised 2017) and Work Experience Sheet 

  3. Certificate/s of Employment from previous employer/s.

  4. Certificate/s of Trainings/Seminars

  5. Copy of Birth Certificate

  6. Copy of Marriage Certificate (if applicable)

  7. NBI Clearance
