4) Certificate of relevant trainings and seminars attended
5) Copy of PRC License, board of rating, CS Eligibility
6) Certificate from School of Masteral units earned or completed if any
6) Copy of duly signed Very Satisfactory (VS) Individual Performance Contract Rating/Summary of Rating of Performance Contract Assessment if any.
Note:
