For Walk-in Applicants:
Interested and qualified applicants are instructed to submit their applications with complete set of documents:
1. Application Letter indicating the position applied for and addressed to:

2. Updated and properly accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form 212, Revise 2017)
3. Updated and properly accomplished Work Experience Sheet (CS Form 212 Attachment)
4. Photocopy of Transcript of Records ( Authenticated)
5. Photocopy of Certificate/s of Employment
6. Certificate of relevant trainings and seminars attended.
7. Photocopy of Board of Rating/ CS Prof Eligibility (Authenticated if any)
8. Copy of duly signed IPCR/Summary of Rating (for Insider only)

Request for extension of submission and application with incomplete documents will not be entertained.
