HEALTH DECLARATION FORM SAMPLE

      It is mandatory in DepEd to provide teacher applicants a Health Declaration Form in taking the English Proficiency Test (EPT) to ensure they are healthy and not showing of any symptoms of the virus. 

     Thus, health and safety protocols for each schools division must be observed in these trying times.

You must provide a certain schools divisions where you're going to take the test above it. 

Other SDOs medical personnel might provide you a Health Declaration Form upon entry of Testing Centers.
